out of scan range

Whenever I try to load a game like Starcraft or Half-Life, I get the "out of scan range". I knot this means that the refresh rate is too high but I'm using NVRefreshTool 2.1, and even when I turn that down to the 60hz, I still get the same thing. Any help would be appreciated.
